feat: bump to v0.5.0 — major deps upgrade

- commander 12.1.0 → 14.0.3
- ora 8.2.0 → 9.3.0
- @types/node 22.19.11 → 25.5.0
- engines: node >=18 → >=20 (Node 18 EOL, vitest requires 20+)

All 124 tests pass, build OK, CLI verified.
