Re-add SPV_EXT_optnone support and migrate from the deprecated SPV_INTEL_optnone extension. The INTEL variant remains registered for backward compatibility in the reader, but the writer now exclusively uses SPV_EXT_optnone. This reverts commit 2bae401.
